Nurse Julie wins award!

All the staff are extremely proud of Julie Walker who today has won the award for "Nurse of the year" in Sutton. She is a credit to our surgery and we are very lucky to have her as part of our team! 

Physician Assosciate Ruari Blake

Unfortunately Ruari will be leaving our practice on 14/6/22 we wish him the best of luck in his new job and we are sure the staff and patients will miss him greatly. We will be employing a new staff member in due course.

CQC Visit:

We will be due another CQC visit in May 2022, please see the panel underneath under the heading "CQC inspections" if you would like to provide any feedback/reviews on your care

Free NHS Health Check:

Are you aged between 40 and 74 and have no long term health conditions? You could be eligible for a free NHS health check. Please call us on 0203 770 6071 for more details.

Physio:

Unfortunately Isabelle will not be working for a few months but we have a new physiotherapist called Barry working every Thursday. Please call us if you feel like you need to see the physiotherapist.

Health Coach:

We now have Anthea working with us who is a Health Coach. She can deal with issues such as weight management, exercise advice and general lifestyle changes. 

Face to face appointments:

Please inform reception when booking if you require a face to face or telephone appointment.

   

Covid 19 Symptoms:

Please do not attend the surgery if you have any Covid19 symptoms

 

Text Messaging:

We use a text messaging service called Accurx. This will be used in future for vaccination bookings, Asthma questionnaires, Collecting other data to add onto your clinical system. The text will come from "GWS" if you have any queries please call us on 0203 770 6071 and we will be happy to help you with anything.  Text reminders are now turned on for appointments.

 

Cancelling appointments:

Please can you contact us on 0203 770 6071 to cancel any appointments you cannot make as we can give this appointment to another patient. We have a voicemail specifically for this so you do not need to hold.

Registration

 

If you would like to join our GP Practice, please enquire at reception. They will ask you for proof of address, and your visa (if applicable) before we make an appointment with our Health Care assistant, so please make sure you bring these along with you.

We are always welcome to new patients, but you must live within our catchment area, map is at reception.

You will then be asked to fill out 2 forms and to bring these to the appointment with you. These forms are available to hand-write at reception.

Please then book an appointment with our Nursing Team and please bring a urine sample to the appointment with you, or alternatively arrive early and do a sample before seeing the nurse.

 

DURING COVID - We are not seeing new registrations face-to-face for the time being. Please fill out the forms as normal and pass them back to reception. We will then register you.

If you are unable to attend the surgery then please call reception and we will happily email a copy of the forms to you
